We don't access the document we are directed to the SFDC login page

Hello Everyone,

The problem is when a user creates a new version of content.
The link is not updated.if we click on link redirecting to sfdc login page.

unable to see the deployement status

While we Run Test in Sandbox it covers 100% , but during the deployment to Production it throws an error that Code Coverage Failure.
Also we are unable to find the status(failed or successed)
